PT. Indonesia Infrastructure Guarantee Fund (Persero) hereby invites Partners to take part in the Public Auction process with Prequalification for the procurement of PT PII (Persero) Health and Life Insurance in 2022 with the scope of work for Inpatient Insurance and Outpatient ASO for Employees and their Families as well as Employee Life Insurance for the Period of 2022 .

Candidate qualifications:

1. Company Qualification: Large Company
2. Business Field: Procurement of Insurance Services
3. Sub-Sector of Business: Health and Life Insurance Services
4. Is an Authorized Agent/Distributor of Insurance Services for both Health and Life, as evidenced by an official agency/distributor letter
5. Having a network of class A hospitals, especially in big cities in Indonesia
6. Have a Large Trading Business License (SIUP) which is engaged in Trading Health Insurance Services, ASO inpatient, outpatient and life insurance in accordance with this procurement issued by the authorized government agency and is still valid.
7. A list of at least 5 (five) clients who use Health Insurance products
8. Client list of at least 3 (three) existing clients of Health insurance program in the last 5 (five) years
9. List of clients using the ASO method in the last 5 (five) years.
10. Submitting a Letter of Interest
11. Submit Qualification Form
12. Registered as a partner of PT PII (Persero) by filling out a form that can be accessed via the following URL:

13. Submitting Administrative Documents including:
      a. Scan of Company Establishment Deed
      b. Scan of Deed of Amendment;
      c. Scan of Ministry of Law and Human Rights Ratification;
      d. Scan SIUP/NIB;
      e. NPWP scan;
      f. TDP scans;
      g. SKDP scan;
      h. Scan the ID card of the person in charge according to the deed;   
      i. Scan of Company Account Book/Account Cover;
14. Submitting financial statements and proof of tax payment for the last year (SPT); and
15. Has paid taxes and submitted tax reports for the last 3 (three) months;

Criteria for Service Providers are having similar work experience in the last 5 years.
